Instrument Repair Service

1. Clean and sterilize the instruments to be repaired

All instruments / equipment MUST be sterilized and shipped in a sterile pouch with an indicator reading of sterility, in accordance with OSHA Standards.

2. Print and fill out your Repair Form

Your Repair Form must clearly state:

• The type + quantity of each instrument (Example : “Scissors x 2”)
• The service requested for each repair (Example : “Needs sharpening”)
• Any custom requests
• Any special instructions
• ** This completed paper form must be sent in the same package as your instruments to be serviced

3. Use a sturdy shipping box with sufficient packing material to protect your instruments

Note: The filled-out REPAIR FORM must be inside the box with your instruments.

4. Choose a secure, trackable service for shipping

Black & Black Surgical is not liable for any instruments that are lost en-route to our facility.
